2019-20 Annual Graduate Review Checklist

     Fill out and use as the first page for your 2019-20 annual review packet, due to the GIC Chair by 4 September 2020

Annual Graduate Committee Meeting Report Form

     Submit to the Zoology Graduate Program annually (after your first year), excluding years when Form II and Form III submitted

Interim Committee Report Form

     Submit to the Zoology Graduate Chair when completing Form I

Graduate Assessment Form

     Submit to the Zoology Graduate Chair when completing Form II and Form III

Personal Leave Form for TAs

    Use this form for sick leave and other personal matters requiring a brief time off. If you wish to take a leave of absence for an entire semester, then you must petition the Graduate Division in advance by clicking here.

 

Master's Plan A (thesis) Forms

Master's Plan A Form I - Pre-Candidacy Progress

     Submit to Zoology Graduate Chair following first-semester interim committee meeting

Master's Plan A Form II - Advance to Candidacy

     Submit to Zoology Graduate Chair following approval of thesis topic by graduate committee, including preliminary Assessment Form (see above)

Master's Plan A Form III - Thesis Evaluation

     Submit to Zoology Graduate Chair following thesis defense, including final Assessment Form (see above)

Master's Plan A Form IV - Thesis Submission

     Submit to Zoology Graduate Program with final thesis submission

Master's Petition to Enroll in GRAD 700F

     Submit to Office of Graduate Education

Master's Petition to Revise Thesis Committee

     Submit to Zoology Graduate Chair to permanently change graduate committee

Master's Petition for Remote Committee Participation

     Submit (early) to Office of Graduate Education for remote committee participation in thesis defense (not needed for remote participation in other graduate committee meetings)

Master's Plan B Form I – Pre-Candidacy Progress

Final Oral Examination for Doctoral Dissertation Defense
